Inserting a new request for medical information
In the Netherlands patients are the owners of their data compiled in the medical files kept by the treating physician. That's why the exchangeprogram Centraal Meldpunt uses the patient as the carrier of his or her own medical history. Special forms developed for this purpose are filled in by the treating physician and handed to the patient to be kept with all other travel documents.

What to do if the patient lost or forgot this form or when additional data are needed?
The patient has to give written permission to transfer data in medical files from one doctor to another. The proper form in  Dutch and Turkish can be downloaded here.
