Relevant medical data at hand
When traveling to another country and staying there for a considerable period of time it is cconvenient to both the diabetespatient and the physician consulted on a medical problem that all relevent medical data are at hand.

Therefore the Centraal Meldpunt developed  a medical passport containing the patients medical history and the latest prescibed treatment. The passport also contains a consentform with which the patient can give his or her permission to transfer extra medical data when they are needed. Patient information with handy tips when traveling with diabetes completes the medical passport.
